About the H2H National Quality Improvement Initiative


About H2H Program

The H2H national quality improvement initiative is an effort to reduce cardiovascular-related hospital readmissions and improve the transition from inpatient to outpatient status for individuals hospitalized with cardiovascular disease.  The H2H project aims to create a rapid learning community where people can share their knowledge and best practices regarding this critical move. 

Goal: Reduce all-cause readmission rates among patients discharged with heart failure or acute myocardial infarction by 20% by 2012

Rather than imposing and advocating specific strategies, the H2H project provides a central clearinghouse of information and tools, building on what others are doing and have done to improve care transitions and reduce readmissions.  H2H focuses on 3 Core Concepts that provide opportunities for improvement:

 (1)  Is the patient familiar and competent with their medication and does he or she have access to them?
 (2)  Does the patient have a follow-up visit scheduled within a week of discharge and is he or she able to get there?
 (3)  Does the patient fully comprehend signs and symptoms that require medical attention, and whom to contact if they occur?
