Background
The H2H Challenges provide the community with recommended strategies and tools to achieve small, attainable goals in their organization. The goal of the H2H Challenges is to evaluate strategies for reducing unnecessary readmission and improve transitions of care for HF and AMI patients; ultimately providing the H2H Community with an online repository of strategies and tools.

Improvement Project Structure
Each Challenge project is six to eight months and consists of success metrics, three webinars, and one tool kit. A collection of best practices from the field is developed for each of the H2H Core Concept Areas: (1) early follow-up "See You in 7", (2) post discharge medication management "Mind Your Meds", and (3) patient self-care and activation "Signs and Symptoms".
H2H Challenges Oversight
Leadership, oversight, and guidance of the H2H Challenges are provided by H2H Steering Committee members and the H2H Mastermind Group of community volunteers.
