 H2H Challenges: A Community Call-to-Action

Background

Since the H2H Initiative launched in 2009, participation in the H2H Community has grown to over 50 strategic partners and thousands of individuals and their respective organizations.  This active community has established their commitment to reducing readmissions and improving transitions of care. In 2011, IHI and the ACC are “challenging” the H2H Community to try recommended strategies and tools and help build a H2H Tool Kit to achieve small, attainable goals in their organization.


Goal of the H2H Challenges

The goal of the H2H Challenges is to evaluate strategies for reducing unnecessary readmissions and improving transitions of care for HF and AMI patients; ultimately providing the H2H Community with an online repository of strategies and tools with information on their implementation.

 

H2H Challenges Oversight

Leadership, oversight, and guidance of the H2H Challenges are provided by H2H Steering Committee members and the H2H Mastermind Group of community volunteers.
