Strategic Partnership 

The H2H Strategic Partnership is a partnership of trade organizations, medical societies, quality improvement organizations, and other healthcare orgnizations that are committed to improving care transitions, empowering patients and ultimately reducing preventable and costly hospital readmissions.

Strategic partners are asked to assist the initiative through various activities, including public support of the goals, active promotion of the initiative and/or recruitment of participating hospitals and practices.

Organizations are asked to support the Partnership by: 
• Providing leadership in reducing hospital readmissions
• Disseminating products and materials
• Promoting the Partnership with member organizations or organizational constituents
• Helping to assess the impact of the H2H Quality Initiative
